Forms, Applications, Permits, etc.

All forms must be downloaded, completed and emailed back to the City Clerk at cityclerk@cityofholden.com .

You can either use the online payment portal at: Online Payment or, you can stop by City Hall to drop off your form and make your payment.  

The forms PLUS any associated payment MUST both be received and reviewed by the City Clerk, and you must receive a confirmation from the city clerk prior to assuming you have the permission, or license, or reservation.  Please allow 3-5 business days for confirmation.  

Please let City Hall know if you experience any issues with any form or application so we can correct it.
